What is Clomid?

Clomid (or as a generic drug, clomiphene citrate) is an fertility medication that comes in a tablet and works to rebalance your hormones. When you take Clomid, your body makes more follicle-stimulating hormone (FSH) and luteinizing hormone (LH). Together, these encourage follicles in your ovaries to develop and that helps your eggs grow.

If your doctor feels Clomid is right for you, you’ll likely start with a dose of 50 milligrams (mg) per day. This dosage can increase by increments of 50 mg if you don’t become pregnant after the first round up to as much as 250 mg. If you still don’t conceive, up to four treatment cycles of Clomid can be repeated.

Along the way, your doctor will check how your ovulation is progressing via blood tests and ultrasounds. The goal? Connecting your egg with that sperm, either with sex or intrauterine insemination (IUI). If you’re still trying to conceive after six months on Clomid, you may need other fertility help, such as hormone shots or in vitro fertilization (IVF).

Who is a good candidate for Clomid?

If your menstrual cycle has been deemed wonky in some way (no eggs, too few eggs or eggs not quite up to snuff), you’re under 35 and you’ve been trying to conceive for more than a year, you could be a good candidate for Clomid. Before you’re given this ovulation enhancer, speak with your doctor about any allergies you have or other medications you take.

What Happens If the Clomid Cycle Fails

If your pregnancy test is negative, your doctor may wait and retest you again. It’s possible the pregnancy hormone levels just aren’t high enough yet. If you get your period while waiting to test again, the cycle probably didn’t work.

Having an unsuccessful cycle can be upsetting, and it’s common to feel defeated and lose some hope. Remember that you can try again and that sometimes treatment needs adjustments before you reach success.

What happens after a failed Clomid cycle? Every case will be different, but here are some possibilities.

Another cycle of Clomid at the same dose you just used. Just because the cycle didn’t lead to pregnancy doesn’t necessarily mean anything about your treatment should be adjusted right away. You just might need another try or two.

Remember that even people with good fertility rarely conceive the first cycle they try. The same could be said about Clomid or any fertility treatment.

If you didn’t ovulate, another cycle of Clomid at a higher dose. The first cycle of Clomid is typically started at 50mg, but many women won’t ovulate at that dosage. If this happens to you, the dosage may be increased.

Your doctor will probably keep trying higher dosage levels up until a certain point, after which other treatments might be considered. Women who don’t ovulate on Clomid are sometimes called Clomid resistant. There are a variety of options if this happens to you, including switching to the drug letrozole or taking Clomid along with metformin.

If three to six cycles of Clomid don’t lead to pregnancy, new treatment options may be considered. Six cycles of Clomid is considered to be the top limit, though some doctors suggest moving on if pregnancy doesn’t occur by the third treatment cycle.
